Show 390 Jerry and Vickie Moyes College of Education • Credit Hour Requirements: A total of 120 credit hours is required for graduation; 40 of which must be upper division (courses numbered 3000 and above). Note: Required support courses and professional knowledge courses will not be accepted folder than 10 years. Required athletic training major courses will not be accepted if older than ,5 years. Advisement The ATN department advisement coordinator will serve as the academic advisor prior to admission to the program, and will assist with advisement through completion of the program of study. Call the ATN department (801-626- 6741) for more information or to schedule an appointment (also refer to the Department Advisor Referral List). It is the responsibility of the student to schedule the required meetings with the advisor. Once students are admitted to the athletic training program, advising for major required courses will be provided by the Program Director, Dr. Jennifer Ostrowski (JennOstrowski@ weber. edu) Admission Requirements 1. Before a student can be considered for the program, the following application requirements must be met: 1. Admission to Weber State University 2. Minimum 2.7 overall GPA 3. Submit Athletic Training Program Student Application 4. Submit Letter o f Applicatio n 5. Co mpletion o f 5 clinical obse rvation hours (within AT 1500) 6. Grade of B- or better in AT 1500 Introduction to Athletic Training (First Semester) 7. Grade of B- or better in AT 3300 Evaluation and Care of Musculoskeletal Injuries: Lower Extremities 8. Grade of B- or better in AT 3301 Evaluation and Care of Musculoskeletal Injuries: Upper Extremities 9. Interview with program faculty 10. Complete Technical Standards form 11. Submit unofficial transcripts from other colleges/universities attended 12. Physical examinatio n and p roo f of immunizatio ns 13. Current Emergency Response and CPR/ AED for Professional Rescuer cards 14. Co mpletion o f the fo Ho wing Pre- requisite Courses with a grade of C or better: ■ AT 2300 Emergency Response (3) HTHS 1110 Integrated Human Anatomy and Physiology I (4) * HTHS 1111 Integrated Human Anatomy and Physiology II (4) * PSY 1010 Introductory Psychology (3) NUTR 1020 Science and Application of Human Nutrition (3) 15- ■ HLTH 1030 Healthy Lifestyles (3) ■ PHYS 1010 Elementary Physics (3) ■ PEP 3280 Methods of Teaching Strength and Conditioning (3) ■ PEP 3450 Structural Kinesiology (3) For students participating in varsity and club sports, a contract must be signed by both the student/athlete and his/her coach. The contract stipulates that AT students may participate in one sport only, beginning with their first clinical year in the AT Program. More details can be found in the AT Program Student Handbook. *ZOOL 2100/2200 (Human Anatomy/Physiology may be substituted for HTHS 1110/1111) NOTE: It is recommended that students not have more than 9 credits of general education requirements remaining prior to admission. Those with more should work with the HPHP departmental academic advisor to develop a plan to complete general education courses. Note: After formal admission to the Athletic Training program, students are required to complete an FBI background check and drug test. The expenses, approximately $75, will be paid for by the student. Application deadline is the third Wednesday in February for consideration for each spring semester. Applicants will be notified of the selection committee's decision based on cumulative WSU GPA through the fall semester. Applications may be obtained from the AT Program Director (JennOstrowski@weber.edu) within AT 1500 only. Students interested in transferring from another institution or from another major at Weber State University must meet University and College transfer requirements and complete the same requirements as freshmen in the Athletic Training Program.
