Show Services of the College College Scholarship Service—-Weber State College participates in the College Scholarship -Service (CSS) of the College Entrance Examination Board. Participants in CSS subscribe to the principle that the amount of financial aid granted a student should be based upon financial need. The CSS assists colleges and universities and other agencies in determining the student's need for financial assistance. Entering students seeking financial assistance are required to submit a copy of the Parents' Confidential Statement (PCS) form to the College Scholarship Service, designated Weber State College as one of the recipients, by February 15. The PCS form may be obtained from a secondary school or the College. FOREIGN STUDENT ADVISEMENT All students from outside the United States are considered Foreign Students. Special on-campus advisement is provided for such students. At the beginning of each quarter Foreign Students are required to register with the Foreign Student Adviser. It is also necessary for them to keep him informed concerning such matters as local address, change of address, change of student status, etc. Requests for permission for employment, immigration certification, extensions of visa, money exchange letters, and transfer of schools, must be submitted through the Foreign Student Office. Assistance is also offered with personal and academic matters through all of the other Student Personnel departments. Foreign Students are urged to consult frequently with their adviser and to keep him informed on their problems and special needs. The Foreign Student Office is located in Annex I, Room 110. HEALTH SERVICE, POLICIES, AND PROCEDURES FOR PHYSICAL EXAMINATIONS The Health Service of Weber State College has been established for the education and protection of students. The services offered are: (1) evaluation of the medical examination given by the family physician, (2) follow-up for the correction of remediable defects, (3) adequate care in case of accidents and sudden illness, (4) procedures to control communicable diseases, (5) a program of accident prevention. A medical examination is required of every student upon his initial entrance to Weber State College. Registration cannot be completed until he has filed a completed report on the official form. The college physician shall prepare the official examination form to be given each initial applicant to Weber State College. A student shall have his family physician (M.D.) give the examination and record it on the Weber State College Medical Exam- 72 Services of the College ination Record Card. The college physician will evaluate the results of the medical examination for the guidance and protection of the students and the college. The college physician shall be the final judge of a student's physical fitness for participating in activities and for using the college facilities. College Physician—Russell N. Hirst, M.D. College Nurse—Margaret Nicholson HIGH SCHOOL RELATIONS High School Relations—The high school relations program is a function of student personnel. It includes visits to all high schools in the state by representatives of the College. The purpose of these visits is to help high school students gain an understanding of educational opportunities beyond high school and to provide specific information concerning Weber State College. Also included is the coordination of on-campus tours of interested student groups and other individuals or groups interested in the programs at Weber State College. STUDENT HOUSING SERVICE On the Weber State College campus there are four residence halls available to house students. Living in the residence halls provides an excellent opportunity for students to live, work, and study together. This becomes a vital part of the college experience. Each hall is under the supervision of a Head Resident couple who are well qualified and trained to work with students. The Head Residents are assisted by a Resident Assistant on each floor. Resident Assistants are usually junior and senior students. A student government organization provides for a well balanced educational, cultural, and social program in each hall. Residents must be regularly enrolled full-time (10 hours or more) Weber State College students. Room assignments are made for the full academic year. Student Housing Policy—All single freshmen and sophomore women students under 21 years of age, not living at home, are required to live in college operated residence halls.
